Hemorrhoids are clumps of dilated (enlarged) blood vessels in the anus and lower rectum. The rectum is the last area of the large intestine before it exits to the anus. The anus is the end of the digestive tract where feces leaves the body.

Sometimes hemorrhoids swell when the veins enlarge and their walls become stretched, thin, and irritated by passing stool. Hemorrhoids are classified into two general categories:

  • internal, originating in the rectum, and
  • external, originating in the anus.

What’s the difference between anal fissures and hemorrhoids?

At first it was something tiny. Almost nothing.

How do I know if I have piles?

First, your doctor will look at the anal area. He or she will likely also insert a lubricated gloved finger and may insert an anoscope (a hollow, lighted tube for viewing the lower few inches of the rectum) or a proctoscope (which works like an anoscope, but provides a more detailed rectal examination) into the back passage.

More procedures may be needed to identify internal haemorrhoids or rule out other conditions that may cause anal bleeding, such as anal fissure, colitis, Crohn’s disease, and colorectal cancer.

To see further into the anal canal (into the lower, or sigmoid, colon), sigmoidoscopy may be used, or the entire colon may be viewed with colonoscopy. For both procedures, a lighted, flexible viewing tube is inserted into the rectum. A barium X-ray can show the entire colon’s interior. First a barium enema is given and then X-rays are taken of the lower gastrointestinal tract.

  • Piles are collections of tissue and vein that become inflamed and swollen.
  • The size of piles can vary, and they are found inside or outside the anus.

Types of piles

Internal piles start inside your anal canal, but they might hang down and so come out your anus. Internal piles can be graded according to whether they come out your anus and – if so – how far they come out

  • First degree piles may bleed but don’t come out of your anus.
  • Second degree piles come out of your anus when you have a bowel movement, but go back inside on their own afterwards.
  • Third degree piles come out of your anus and only go back inside when you push them in.
  • Fourth degree piles are always partly outside your anus and you can’t push them back in. They may become very swollen and painful if the blood inside them clots.

External piles are swellings that develop further down your anal canal, closer to your anus. They can be very painful, especially if they have a blood clot in them.

Piles is classified into four grades:

  • Grade I: There are small inflammations, usually inside the lining of the anus. They are not visible.
  • Grade II: Grade II piles are larger than grade I piles, but also remain inside the anus. They may get pushed out during the passing of stool, but they will return unaided.
  • Grade III: These are also known as prolapsed hemorrhoids, and appear outside the anus. The individual may feel them hanging from the rectum, but they can be easily re-inserted.
  • Grade IV: These cannot be pushed back in and need treatment. They are large and remain outside of the anus.

External piles form small lumps on the outside edge of the anus. They are very itchy and can become painful if a blood clot develops, as the blood clot can block the flow of blood. Thrombosed external piles, or hemorrhoids that have clotted, require immediate medical treatment.

Hemorrhoids or Piles

Hemorrhoids are caused by swelling in the anal or rectal veins. This makes them susceptible to irritation.

This swelling can be caused by several things, including

  • obesity,
  • pregnancy,
  • standing or sitting for long periods,
  • straining on the toilet,
  • chronic constipation or diarrhea,
  • eating a low-fiber diet,
  • coughing,
  • sneezing,

  • holding your breath while straining to do physical labor.

  • Eat plenty of fibre-rich foods such as fruit, vegetables, wholegrain cereal, wholegrain rice and pasta, nuts, beans and pulses. Aim to eat 25 to 30g of fibre a day. As a guide, half a cup of bran cereal contains nearly 10g of fibre and a medium-sized apple has about 3.5g.
  • Drink plenty of fluids. Try to limit your intake of caffeinated drinks.

Complications of piles

Piles rarely cause serious problems but they can lead to some complications, such as those below.

  • Ulcers can form on external piles.
  • Skin tags can occur when the inside of a pile shrinks back but the skin remains. For more information about skin tags, see our FAQs section.
  • Mucus leaking from your anus can cause the surrounding skin to get very sore and damaged.
  • Severe piles (fourth degree piles) can sometimes get strangulated, meaning they lose their blood supply. This can be very painful and cause the tissue to die.

Bleeding during bowel movements is the most common sign of hemorrhoids. Your doctor can do a physical examination and perform other tests to confirm hemorrhoids and rule out more-serious conditions or diseases.

  • switch to wholemeal (or granary) bread and pasta
  • eat baked rather than boiled potatoes – and don’t leave the skins on your plate!

  • If you’re tempted to have a drink of fruit juice, try the real thing instead. Fruit can quench your thirst, but has the added benefit of plenty of fibre
  • Snack on dried fruit rather than reaching for the biscuit barrel
  • drink enough fluid (2-2 ½ litres, or 6-8 cups, a day).

Who gets piles?

Piles can occur at any age, but are more common in older people. They affect both men and women. In fact, most people suffer from piles at some time, but usually they are nothing more than a temporary problem. Many experts believe that they are caused by continuous high pressure in the veins of the body, which occurs because humans stand upright. They are particularly common in pregnancy because of the additional pressure from the baby, and because of hormonal changes. Sometimes they result from straining hard to pass faeces, which is more likely if you do not eat enough fibre, or lifting heavy weights. They are not caused by sitting on hot radiators or cold, hard surfaces, or by sedentary jobs. A family history, as with many other health-related problems, is frequently found and is likely to be a significant risk factor for developing piles.

Anal Fissures

Unusual bowel movements, including hard stools and diarrhea, can cause the tender lining of the anus to tear. These tears are called anal fissures. In addition to constipation and diarrhea, other risk factors for fissures include having inflammatory bowel disease, being overweight or being of an advanced age.

Anal fissures are painful. They especially hurt when passing stool, and the pain can last for hours afterward. It is often experienced as a throbbing sensation.grater toilet roll

Fissures can also itch uncomfortably. Plus, you may notice blood when you wipe or spot blood in the toilet bowl.


Hemorrhoids are often accompanied by fissures and show many of the same symptoms. . The difference between anal fissures and hemorrhoids is that they are each a different kind of injury. While an anal fissure is a tear of tissue, hemorrhoids are caused by the weakening of cushions of tissue in the lower rectum, causing the skin to blow up like a tiny balloon and fill with blood[A1] . There are two kinds of hemorrhoids – external hemorrhoids and internal hemorrhoids. Pain in the rectum is usually associated with external hemorrhoids and/or a fissure. Uncomfortable, sure. Harmful? Not so much

Okay, so that description certainly sounds miserable, but the faint glimmer of good news here is that hemorrhoids aren’t going to cause any direct harm to your health. They’re not related to cancer in any way (phew). The only cause for serious concern is if they’re bleeding excessively, which could lead to anemia, Grucela says, but that’s pretty rare.

Constipation is a major trigger

All that straining and pushing increases the pressure that can lead to inflamed hemorrhoids. Treating any constipation is the second step in standard hemorrhoid treatment, Grucela says, and the easiest way to do that is getting hydrated, fast. The more parched you are, the more parched your poop is. Eating enough fiber (or taking a fiber supplement, whether in old-school powder form or newfangled gummies) helps bind water to stool. “That way, it will be softer and pass through less traumatically,” she says. The ideal: The kind of poop that glides out so effortlessly you almost don’t even need to wipe. What is Anal Itching?

Also called puritis ani, anal itching it’s usually not deadly but can be intensely irritating and downright painful. Not only does the anus itch but often the skin around the anus called the perianal skin. The bad news is that in most cases doctors cannot find a cause for anal itching in their patients. The good news is that over the counter and home remedies often provide effective relief.


Known causes of anal itching include lack of bathing, hemorrhoids, parasite infestation, tumors growing in the anus or allergic reactions to soaps, medications, undergarments or personal cleansing wipes.

Below are some practical tips to help with the treatment and prevention of an itchy bottom:
Home remedies and self-care

  • Avoid highly seasoned and spiced foods, and maintain a regular bowel habit
  • Keep the anal area clean after emptying the bowel
  • Avoid medicated, perfumed, or deodorant soaps – use only plain warm water for cleaning, aqueous cream, or emollients
  • Use moist toilet tissues instead of paper

When to see a doctor

People who have anal itching plus bloody diarrhea or draining pus should see a doctor in a day or two. Other people should see a doctor if the itching has lasted for more than a few days, but the visit is not urgent.

What the doctor does

Doctors first ask questions about the person’s symptoms and medical history. Doctors then do a physical examination. What they find during the history and physical examination often suggests a cause of the itching and the tests that may need to be done (see Table: Causes and Features of Anal Itching).

The history is focused on when the itching started and how long it has lasted. Doctors ask about the following:

  • Ingestion of irritating foods, particularly acidic or spicy foods
  • Bowel habits, including use of wipes, ointments (even those used to treat itching), sprays, and soaps applied to the anus
  • Hygiene habits, particularly frequency of showers and baths

  • Known infections or disorders (such as diabetes, hemorrhoids, or psoriasis Doctor

  • Recent use of antibiotics

The physical examination is focused on the appearance of the anus and the perianal skin. Doctors examine this area for

  • Dullness and thickness
  • Signs of irritation due to scratching

  • Hemorrhoids, lesions, fistulas, and scrapes (caused by scratching and rubbing)

  • Scabies or pinworms

Is Bleeding From the Back Passage Serious?

Bleeding from the anus can be serious if the bleeding is connected to a bowel disease or condition rather than the anus itself. These diseases/conditions include:

  • Cancer of the rectum / lower colon
  • Innocent or benign tumours (polyps)
  • Inflammation of the rectum (proctitis)
  • Inflammation of the colon (colitis)

Your GP will help to evaluate your problem and may use the following procedures when examining you:

  • Examine your abdomen, searching for lumps or tender areas
  • Examine your back passage to check the anal canal

Following these investigations, your GP may decide to send you to a specialist who may investigate further and you may need to undergo some common tests and procedures.

Hemorrhoids Treatment and Prevention (Hemorrhoids Piles Treatment)

Rubber band ligation

This procedure is often done on prolapsed hemorrhoids, internal hemorrhoids that can be seen or felt outside. Using a special tool, the doctor puts a tiny rubber band around the hemorrhoid, which shuts off its blood supply almost instantly. Within a week, the hemorrhoid will dry up, shrink, and fall off.

Coagulation or cauterization

With an electric probe, a laser beam, or an infrared light, your doctor will make a tiny burn to remove tissue and painlessly seal the end of the hemorrhoid, causing it to close off and shrink. This works best for prolapsed hemorrhoids.

A high-fibre diet with large amounts of water is the answer for grade I internal hemorrhoids and painless external hemorrhoids. This will soften the stool, decreasing constipation and straining. It will also allow the inflamed veins to decrease in size. There are also a number of ointments and suppositories available without prescription that can help reduce pain and inflammation around the anus. These include topical hemorrhoidal preparations that contain local anesthetics for their soothing properties. Other useful measures include stool softeners or bulking agents, or a sitz bath, used 3 or 4 times daily for 15 minutes at a time. What is a Chronic Anal Fissure?

A chronic anal fissure is a small tear in the thin, moist tissue (mucosa) that lines the anus.

A Chronic Anal Fissure may occur when you pass hard or large stools during a bowel movement. Chronic anal fissures typically cause pain and bleeding with bowel movements. You also may experience spasms in the ring of muscle at the end of your anus (anal sphincter).

Chronic Anal Fissures are very common in young infants but can affect people of any age. Most chronic anal fissures get better with simple treatments, such as increased fiber intake or Sitz Baths. Some people with chronic anal fissures may need medication or, occasionally, surgery.

Signs and symptoms of a chronic anal fissure include:

  • Pain, sometimes severe, during bowel movements
  • Pain after bowel movements that can last up to several hours
  • Bright red blood on the stool or toilet paper after a bowel movement
  • Itching or irritation around the anus
  • A visible crack in the skin around the anus
  • A small lump or skin tag on the skin near the chronic anal fissure


Personal Experience

For the record I was a chronic anal fissure sufferer myself and dealt with over a year’s of chronic pain until I realized a few things during my experience but mainly is that you have to learn to manage your chronic anal fissure verses focusing on trying to cure it. I couldn’t sit, walk, sleep, lie down without being in excruciating pain. A number of people will have a mild chronic anal fissure and it will cure within 2 weeks (you are the very lucky bunch).

I suspect if you here reading this, you are a chronic anal fissure sufferer like myself asking why nothing seems to work and why the insufferable pain continues. I have tried many home and doctor-directed remedies without success, nothing seemed to work. I saw two Proctologists who had different ointments and methods for treatment. I will talk about these and the various home remedies, sitz baths and the various things I tried and hopefully, some of them will help you.


Acute vs Chronic Anal Fissure

90% located in the posterior anal canal. Acute vs Chronic. Acute: small tears in anal mucosa. Usually, respond to medical treatment. Resolve within 4-6 weeks. Chronic: present for longer periods of time. Associated with sentinel tags, rolled mucosal edges and hypertrophic anal papilla at fissure apex. Less likely to heal with medical management.

Before we get too deep into the article, there were three main things that I felt helped me in the long term.

1) I learnt to drink a lot of liquid once I realized it helped me pass stools easier. It was a bit of an epiphany because it was so obvious that I missed it. I was more focused on getting stool softeners that I missed the obvious.

2) I was taught various exercises by an Osteopathy Centre who practises Osteopathy, one of those excesses which I will call The Table Method which best describes it albeit not the proper name (Basically the just of it is treat your butt like a table when you sitting) helped me a lot as I didn’t realize I was not. I will talk later about exercises, but keeping your butt stretched out at all times whenever you sit even if painful, over time strengthen muscles and reduced the pain.

3. A medication that lowers your blood pressure. Hear me out, the way this is supposed to work is it causes your blood pressure to heal in other places. As a not the ointments prescribed but your doctors do exactly the same thing but in the form of a lotion or paste. If your doctor did prescribe something to you, he/she likely told you that you would experience headaches basically because it is lowering your blood pressure.

The tablets I took did the same thing but you swallow them, a heck of a lot nicer to apply. The medication I am talking about is Diltiazem 180MG. Speak to your doctor about it and discuss whether it is right for you. I know you going to be hesitant to try take this, but this basically got rid of the pain. It was prescribed by an experienced pain doctor at one of the largest hospitals in the area and here is a reference on webmd.com



The two other creams that I tried, but didn’t actually work for me, but may you. Chances are you already have been prescribed one or the other


Here some products you may want to consider trying, some may work, some may not. Personally, I have tried the Honey and Coconut Oil. It’s going to vary from person to person and may work for you before you rule it out.

    Water, Water, Water …


    It’s incredibly important to drink lots of fluids. I know it is very difficult consuming water (at least for me), so if you can’t consume water, consume a liquid that you do like such as Fruit Juice even if it is unhealthy for now. Try to consume more health conscience liquids if you can, but bottom line, consume as much liquid in a day as possible, this can be Water, Soup, Juice, Smoothies, avoid things like alcohol as it dehydrates your body or takes in moderation just like anything else.

    Stool Softeners and Liquid Intake

    *This is important to understand & read*

    Let’s talk about stool softeners and if they are harmful to you long term. The risk of problems from long-term use of stool softeners such as docusate sodium (Colace, Regulax SS, other brands) really isn’t the issue. The question is whether they do much good. Studies have cast doubt on the effectiveness of preparations taken expressly for stool softening. Besides, there are plenty of other ways to accomplish the same thing. The difference between a hard stool and a soft one is its water content.

    I personally felt stool softeners did not work well at all, when I initially took them, they worked for the first little while and then stopped work. They expensive and personally I think not healthy to take long time. It also does not help your chronic anal fissure recover if you constantly taking stool softeners, avoid. But if you need to take them, then use some initially and then stop.

    Increasing your fiber intake can help soften stool because fiber pulls water into the intestine. Many of us get only a small fraction of the 20 to 35 grams of fiber we’re supposed to eat each day. Fruit, vegetables, and whole grains are good sources. Fiber can produce abdominal cramps, bloating, and flatulence, so it’s best to increase your intake nice and gradually.

    You body will get climatized to stool softeners and they really aren’t effective for longer than a week or so. This is the important part and took me a very LONG time to realize this. The secret to soft stool is drinking lots and lots of liquids, this can be water or this can just be liquids that you enjoy. I found it difficult to drink 8 glasses of water a day. I tried various things to try remind me to drink water. I put a water bottle on the desk and even though it was in front of me, I still didn’t feel like drinking it. It really doesn’t matter what liquids you drink, just drink liquids.

    Some examples of liquids are

    • Milk with your cereal
    • Water (keep it to half a glass, you don’t have to force yourself to drink a whole glass, this will make it easier, but do try drink at least 3 glasses or a more a day if possible but break it up in smaller doses)
    • Fruit Juice
    • Healthy Fruit Smoothies
    • Tea (Herbal / Caffeine free is best)
    • Coffee (try avoid too much coffee as it has caffeine and is supposedly not good for chronic anal fissures). 1 – 2 Cups a day in moderation should be fine. 

    All of these will ultimately help your stool soften. Bear in mind it takes while to become climatized to liquids, but stick with it, try drink as much liquid as you possibly can. This is what I typically do, but try drink more if you can, at minimum you should be drinking in the Morning, Noon and Dinner.

    My liquid schedule and this made things a lot better for me

    • Wake up in the morning before breakfast (Drink half a glass of water). I leave a glass on the kitchen counter to remind me to drink water when I see the glass.
    • Eat Cereal (Filled with half a glass of milk or more)
    • Lunch I have a glass of Happy Planet Smoothie (find something you enjoy drinking even if it is not water)
    • Drink half a glass of water in the afternoonHappy Planet Green Smoothie
    • Dinner I have a glass of the Happy Planet Smoothie or Soup
    • After dinner, I have a cup of Tea before bedtime
    • After I wash my teeth, I have another half a glass of water and just before bed, I may have some more water.


    I found if I broke up the routine of forcing myself to drink liquids it was easier to consume. The main point here is to make sure you drink LOTS and LOTS of liquids and I promise you, if you do and keep it consistent, it will keep your stool soft and things will run smoothly and become easier over time.

    Are You Exercising? It Can Help You ‘Go’

    We’ve covered the importance of nourishing a healthy gut environment with beneficial bacteria, fermented foods, and sugar avoidance, while eating plenty of fiber-rich vegetables. This is a large part of the picture, but it’s not the whole thing. Exercise is also important for regularity, in part because your abdominal and intestinal muscles work together to help move your bowels. If these muscles are weak, you’ll have a harder time with bowel movements. In addition, movement helps stimulate circulation and intestinal function, causing your bowels to move properly.


    I know you are probably saying to yourself, “Exercise?  Does he know just how much pain I am in?” Or something along those lines.  I have to tell you though, it was probably the lack of exercise that caused my own health problems in the first place.Walking Exercise

    What I have learned is that it is never to late to start exercising and it really does benefit your overall health, both physically and mentally.  Now, since you have an chronic anal fissure your options when it comes to exercising are somewhat limited, but there are still plenty of exercises you can do that will aid your body in the healing process.

    Exercise promotes better cardiovascular health, which helps your heart pump more blood to your bodily organs.  This is so important as your anus is one area of your body that does not get as much blood flow as other areas.

    The reason doctors prescribe Nitroglycerin is because it helps draw blood to your anus when applied, which in return is supposed to help your body heal faster.  I can tell you from my experience using Nitroglycerin ointment that it did ABSOLUTELY NOTHING but give me a headache each time I used it!

    It is important to note that not all forms of exercise are advisable while having an Chronic Anal Fissure.  Here is a list of some recommended exercises and those that you want to avoid.  As with any form of exercise, it is always important for you to check with your doctor before starting any exercise program, especially under these circumstances.

    Good exercises for you while healing from your chronic anal fissure:

    • Walking
    • Yoga
    • Kayaking/Canoeing
    • Stretching
    • Doing some light housework or gardening

    Exercises to avoid while healing from your chronic anal fissure:

    • Biking
    • Heavy weight lifting
    • Contact sports or ones that require a lot of running
    • Any form of exercise that will cause you to strain in performing the exercise

    The Table Method (important)

    One thing that help me a lot, when I would sit for long periods of time, I would feel my butt feel like there is a lot of pressure on it and cause additional pain. Probably because you suffocating all the blood vessels. Try not let your buttocks tighten up whenever you sitting pull your checks apart. Initially this is going to be painful, but you need to treat your butt like a plateau or table. It MUST be flat and spread out, over time if you continue doing this, it will strength your muscles, the idea is to get your buttocks back to normal.

    Pull your butt cheeks apart in the opposite direction from your body. Left cheek to the left and right cheek to the right. Keep them stretched the entire time while you sitting.

    How to do this ? Wear lose clothing when at home, this method doesn’t work well with jeans, but I have done it with jeans either by shifting around or reaching in with my hands and pulling my left cheek to the left and my right cheek to the right. Do this every time you sit down, you will feel it stretching, and just think about keeping your butt flat like a table, sit up straight. If you in a little bit of agony, you doing it right. Keep it stretched. If you in public and can’t put your hands in your pants, then just shift you butt on the chair and use your butt to pull your cheeks apart, this works with jeans.

    Round Medical Seat Cushion Also consider getting additional pillows or a ring cushion to sit on when you are sitting if you still in pain, this will also help. I personally sit on a pillow even today and it does make a huge difference. Avoid sitting on a hard chair if possible and always sit on something soft when you are at home. Even though my couch is relatively soft, I still find it hard to sit on for long periods of time. If you have a nice soft couch or chair, you may not need it, but if not either use a regular pillow or a ring cushion to avoid too much pressure.

    Squatting Can Help if You’re Constipated

    The last thing most people think about when using the bathroom is position, but this can significantly impact the ease with which you eliminate and even increase your risk of bowel and pelvic problems, including constipation, hemorrhoids, and more. Most of you reading this probably sit to evacuate your bowel, but this requires you to apply additional force (straining), which has some unwanted biological effects, including a temporary disruption in cardiac flow.

    Sitting on a modern toilet is designed to place your knees at a 90-degree angle to your abdomen. However, the time-honored natural squat position (which is still used by the majority of the world’s population) places your knees much closer to your torso, and this position actually changes the spatial relationships of your intestinal organs and musculature, optimizing the forces involved in defecation.

    Squatting straightens your rectum, relaxes your puborectalis muscle, and allows for complete emptying of your cecum and appendix without straining, which prevents fecal stagnation and the accumulation of toxins in your intestinal tract. It is instructive that non-westernized societies, in which people squat, do not have the high prevalence of bowel disease seen in developed nations; in some cultures with traditional lifestyles, these diseases are uncommon or almost unknown.

    If you have trouble with bowel movements, especially constipation, I urge you to give the squat position a try. Squatting does involve strength and flexibility that adults tend to lose over time (but children have naturally). Special toilets and stools that get your body into a more “squatty” position can help you get closer to the ideal even if you’ve been sitting for decades.

    Food Allergies


    Another interesting perspective is that chronic anal fissures are related to food allergies. Here is some information I got from my doctor which I will share with you. If nothing is working and you want to try cutting back, another suggestion is to do the following. You should do this consistently for 12 weeks for it to be effective.


    Anal Skin Care

    The following instructions will help reduce or eliminate anal pain, soreness and itching. These instructions are directly from my Proctologist. If you use them, it will only work if you follow them exactly without any short cuts for 12 weeks.


    1. Take 1/2 teaspoon (only) per day of Metamucil or a similar product to start for one week and then increase to 1 teaspoon  per day. Metamucil is available without prescription at most pharmacies. Try to drink some extra water daily
    2. Eliminate the following items from your diet for at least 12 weeks
      • Coffee (including decaffeinated)
      • (you may have to taper gradually) in order to avoid headaches if you a heavy coffee drinker)
      • Tea (decaffeinated teas are permitted, avoid “mint” teas and green teas)
      • Chocolate 
      • Cola Beverages (Coke/Pepsi)
      • Alcohol – Of any kind
      • Citrus fruits and juices, especially grapefruits. Oranges, lemons, limes are not quite as bad
      • Avoid Grapes and grape juice. Do not take prune juice.
      • Apple and cranberry are ok
      • Chronic anal fissures and irritation are more common in summer as patients consume large amounts of seasonal fruits / berries / cherries etc. Please limit these items.
      • Milk, ice cream, yogurt. Cheeses seem less of a problem
      • Vitamin C tablets
    3. Do not use soap or a washcloth to clean the anal skin
    4. When wiping the anus after a bowel movement be extremely gentle.  No vigorous wiping
    5. keep tissue paper (Kleenex) in between the buttocks up against the anus and change it every few hours to keep the anal skin dry
    6. Avoid hot baths and showers. Take a lukewarm (5 minutes only) bath or shower daily.
    7. Wear 100% cotton underwear. Do not wear tight-fitting clothing


    Your symptoms will not resolve overnight. Be Patient. Two to three months may be required before symptoms resolve.




    Sphincterotomy (Surgery)

    This should be done as a last resort and all other alternatives should be tried. This is a painful procedure and not done without considerable thought. You will be in pain for a few days after the surgery.


    A Chronic Anal Fissure is a tear in the skin of the anus, usually caused when a large, hard stool overstretches the anal opening and damages the fragile skin. It is often associated with constipation, and less frequently, with chronic diarrhea, inflammatory bowel disease or sexually transmitted diseases (STDs) that involve the anus and rectum. Sometimes, the cause is unknown.

    Anal fissures can be acute or chronic. An acute fissure is a superficial tear and a short-term condition. A chronic anal fissure is deeper and may expose underlying muscle. When lifestyle and diet changes and medication fail, and the condition becomes chronic, surgery is recommended. A lateral internal sphincterotomy is a surgical procedure most commonly used to repair a fissure.

    A lateral internal sphincterotomy involves stretching or cutting the internal sphincter, the muscle that restricts and relaxes when stool passes from the body. Cutting the muscle prevents spasm and temporarily weakens the muscles, and this in turn helps the area to heal. The surgeon removes the chronic anal fissure and any underlying scar tissue.
    The surgery can be performed under local anesthesia, which involves injecting the immediate area with a numbing agent similar to that used at the dentist’s office, or spinal anesthesia, which numbs the entire lower body. In some cases, general anesthesia may be used, which renders the patient unconscious. The choice of anesthesia depends on patient and physician preference, the patient’s health, and the standard practice of the particular facility.Anal Fissure Header

    The patient is positioned on the table so that the anus and rectal area are exposed. This can be done face down, with the buttocks slightly elevated, or supine (on the back) with the legs held up in stirrups. After anesthesia is administered, the area is cleaned with an antiseptic solution.

    The operation takes about 20 to 30 minutes. The wound is then sutured closed, usually with stitches that dissolve in time. Most patients can go home the same day.

    Sphincterotomy Postoperative Complications

    The risk of complications from this procedure is very low, but they do develop occasionally. Most common are:

    • Excessive bleeding
    • Fecal incontinence, which is the loss of the ability to control bowel movements
    • Inability to urinate
    • Infection
    • Problems resulting from anesthesia
    • Risk of abscess or fistula

    On centrifugal juicers the blade and basket spins at a very rapid speed of around 3,000 – 7000 RPMs (Rev’s per minute) to separate the juice from the pulp. This type of juicer cuts up the fruit and vegetables with a flat cutting blade. The blade has raised sharp little triangular protrusions on the blade which cause the fruit and vegetables to be ripped into little pieces. The sheer power of the centrifugal force causes the juice to be extracted. This is a similar way a washing machine spins to remove water from wet washing.

    The models that don’t come with a pulp ejector has a strainer basket which you will need to empty, around every 2 glasses of fruit juice that the juicer creates.     Advantages of a Centrifugal Juicer

    You probably know by now, that fresh juice is good for you.  But if you are like most Americans and others in the world you are more than likely using a centrifugal juicer to extract your juice.

    Here’s the truth: you are not getting the nutritional benefits you could be if you juiced those same fruits and vegetables with a gear juicer. However, you are getting at least 80% of the nutritional healing benefits of the juice extracted by a gear or masticating type juicer. So that’s not bad…and it’s certainly better than any pasteurized juice you can buy in the store.

    How does a masticating juicer work?


    A masticating juicer works with a single gear or auger.  An auger is a drilling device, or drill bit, that usually includes a rotating helical screw blade called a “flighting” to act as a screw conveyor to remove the drilled out material, or in this case, pulp.  The remaining juice is collected and strained through wire mesh and collected.  As the auger turns, the inserted fruits and produce is crushed, squeezed against the outside wall of the juicer.  The filter, screen or wire mesh which lines the side of the wall holds the pulp while the remaining juices are forced out.  Since this process is done slowly, the juice extract is not heated up.  Nutrients and enzymes at this lower temperature are able to survive and kept intact within the juice.


    What are the benefits of a masticating juicer?

    Fruit platterThere are multiple benefits of using a masticating juicer such as the auger turns and crushes your fruits and vegetables, the slow but efficient process is able to yield more juice from the produce inserted.  This benefit of using a masticating juicer will be evident in the drier pulp.  It can be noted that on average, a masticating juicer will produce about 15% to 20% more juice.  With the rise in the cost of fruits and vegetables, this may be a more economical solution in the long run.  Another benefit of using a masticating juicer is that the nutrients, enzymes and trace minerals are not exposed to the heat of a faster juice extraction process.  This slower process means less heat and less foam in your juice.  By crushing and “chewing” the produce inserted into the juicer, the masticating juicer keeps all of the healthy benefits of your produce intact.  Masticating juicers are also extremely efficient at juicing fruits, vegetables, including leafy greens, grasses and leaves.  Produce like spinach, wheatgrass, celery, parsley, lettuce and other leafy greens and herbs are able to juiced using a masticating juicer.  Something of note that should be considered is that the Champion Juicers are not too great at juicing wheatgrass or other leafy greens.  As an appliance, the masticating juicer is also quite versatile in the kitchen.  Due to the slow spin of a single gear, a masticating juicer will also homogenize foods to make sorbets, butters, ice cream, baby foods, pates and sauces.  Some masticating juicers are so versatile they’re able to create bread sticks and pastas.  The final benefit of masticating juicers is that due to the slower rate of rotation of the single auger, the juicer will last longer.



    Triturating Juicers, their benefits and weaknesses

    Triturating juicers are more commonly known by the name “Twin Gear Juicers” or “Double Auger Juicers”. “Triturate” means to crush or grind into a very fine powder and that is exactlty what these juicers do. Using two augers this juicer type does the best job at juicing for very high yield because it leave the pulp of the juice fully extracted of juice.

    The benefits of triturating juicers are:

    • They produce very high quality juice full of nutrients
    • They create little to no oxidation of the juice due to low speeds
    • They juice vegetables and leafy greens extraordinarily well

    The weaknesses are:

    • They can take a long time to clean up.
    • Juicing can take up a long time when compared to a faster spinning juicer
    • The cost is higher than most other juicers

    The problem with fruit juice…


    Lack of fibre is the key problem. Juicing releases the sugars in fruit and removes the insoluble fibre; blending also releases the sugars and tears apart theinsoluble fibre. Most of the sugar in fruit is fructose, which can only be processed by the liver. A small amount of fructose, in an apple for example, does us no harm because we consume it along with the fibre. Fibre protects us against the effects of fructose by slowing its absorption, and also makes us feel full. Fruit juice, on the other hand, is absorbed immediately, like all sugary drinks, as the fibre has been removed.



    There are many important reasons why you should juice. I’m not advocating that you should ONLY juice all your fruits and vegetables.  No, you must still eat your fiber too.  What I’m suggesting is that you drink fresh juices on top of your fresh foods, for healing purpose.  This site is about juicing for health and drinking fresh juices provides highest absorption of the best nutrients for rapid healing. Here are only some of the reasons:

    • Fruit juicing helps to “pre-digest” your food.
      As a result of our poor food choices over the years, most of us have bodies which have become weak in absorbing the nutrients that we need. Juicing “pre-digest” the fruits so that they are readily digested and assimilated before it gets eliminated from our body.
    • You will need to eat one or two pounds of fruits daily to obtain the optimum level of nutrients needed for your body.

    If you’re just starting out with juicing, the first thing you’ll need is…well…a juicer! There are two main kinds of juicers out there—centrifugal (AKA: fast) and masticating (AKA: slow or cold-press) juicers.

    The typical juicer you grab from the big box store is probably a centrifugal juicer. The disadvantages of these kind of juicers are pretty numerous—they aren’t very efficient at extracting juice, they don’t do well juicing leafy greens, they are loud as all get out, and some of these juicers get hot enough that they actually break down of the nutrients in the juice.

    The advantage of these kind of juicers? They are very affordable—you can grab one for about $40. If you only want to juice occasionally and not for nutrition purposes (like say for cooking or baking), a centrifugal juicer will do you fine.


    One important note: If you consume grapefruit juice, keep in mind that it can negatively interact with some medications, such as statin drugs. If you have any concerns, check with your doctor to see whether you can safely consume the juice.
    Bottom line? If you enjoy juice, go for the real thing, and limit yourself to one 8-ounce glass per day — or 4 ounces if you’re limiting calories or sugars.

    There is a whole discussion around the negatives of 100% percent juice, take note this is the juice you can buy from the store and in a lot of cases is concentrate.  Nutritionist Dr Jonny Bowen suggests removing this type of fruit juice from your diet entirely. Juicing itself is a healthy way to consume the right type of juice. The 100% Juice is mostly just pure sugar which is not what you want and has no real nutrition value. According to Medline Plus, consuming juice before a meal can actually make you more hungry because it adds calories and could cause you to eat more than if you had not had it. A cup of apple juice only has 0.2 gram of fibre, a cup of fresh raw orange has only 4.3 grams of fibre. A whole apple contains 3.3 grams of fibre.


    A few other tips for juicing

    If you do decide to take the plunge, here are a few suggestions we’ve found make the best juicing experience:

    • Focus on vegetables, but include some fruit. We’ve found that one small apple makes a huge difference in flavor. This is all we recommend so far in the fruit department.
    • Include 1-2 root vegetables–such as carrots or beets–to add additional flavor and antioxidants. We like to include either 2 small carrots (1 large) or 2 small beets (1 large). These tend to contribute a tad more sugar and a nice earthy flavor, adding complexity.
    • Add at least one leafy green vegetable. We’ve been happy with kale, broccoli and chard (currently in season), but others likely work as well.
    • Include one watery vegetable. Cucumber, celery and other watery vegetables help dilute the too-strong flavors of the previous 3 ingredients. This can make a big difference in drinkability, while adding some nice flavor and additional vitamins.
    • Don’t forget a garnish. A small amount of a strong flavored item such as ginger, lemon, mint or other herb can make your juice a real pleasure to drink while adding highly concentrated antioxidants.
    • Consider adding back a few scoops of the fiber that is filtered out by the juicer. This will make your juice more filling.



    You can’t go wrong with any of these Juicers even the low budget one, all of these are good choices and you won’t regret buying any of them. I would recommend you focus more on veggies than fruits, but as long as you not taking too much of one thing, you will be fine.
